Central Serous Chorioretinopathy: Symptoms, Causes and Treatment

A sudden loss of clarity or distorted area in central vision may be a cause for worry even when the eye appears normal. CSCHR is a retinal issue in which fluid accumulates under the macula which is the retinal section responsible for sharp central vision. Also known as central serous chorioretinopathy it also goes by the abbreviation. CCSR which at times is used. Persistent or recurrent fluid requires attention from an ophthalmologist.

What Is CSCHR?

CSCHR which is a condition that results from fluid from the blood vessel rich layer under the retina, the choroid passing through the retinal pigment epithelium and accumulating under the retina. This may cause the macula to slightly detach which in turn affects detail vision. Also reported on the NCBI Bookshelf clinical overview is that this disease may cause blurred vision, distortion and that objects appear to be of a smaller size.

In the course of daily life central serous chorioretinopathy may cause issues with reading, recognizing faces, or seeing fine details. The condition usually affects one eye, that said retinal changes may also present in both.

Symptoms of CSCHR

People that have CSCHR may see blurred central vision, straight lines which appear wavy or bent, a dim or gray patch in the center of what they see, color which is less bright and not as vivid. Also these symptoms can appear in other retinal conditions which is why a professional eye exam is required for diagnosis.

SymptomWhat it may feel like
Central blurFine details appear less clear
DistortionStraight lines may look bent or wavy
Grey patchA fixed dim area appears centrally
Reduced contrastColours or objects look less vivid

CSCHR Causes and Risk Factors

The cause of csr eye disease causes is not attributed to a single factor. Research reports that we see changes in choroidal circulation and increased vascular permeability under the retina. Also associated with Corticosteroid exposure and other health issues is CSCHR has been reported. Stress is a discussion point but it is not the only cause.

If at any time a person has developed CSCHR while on prescribed use of steroid tablets, inhalers, nasal sprays, injections or creams then that should be brought to the notice of the ophthalmologist. Sudden cessation of steroids is not to be done without the go ahead from the health care provider who has prescribed them. In the evaluation of csr eye disease causes we do a review of the patients medications, general health, past episodes and retinal reports.

How Is CSCHR Diagnosed?

A retinal exam is recommended when CSCHR is present. We use optical coherence tomography which we also know as OCT very often, it’s because this equipment is able to identify fluid under the retina and also to see if there is improvement. In some patients also we do fluorescein or indocyanine green angiography.

At Vasan Eye Care we have retina test which Retina at Vasan Eye Care and specialist evaluation play a role in distinguishing this condition from other macular disorders.

TestWhy it may be used
OCTDetects and monitors subretinal fluid
Dilated retinal examinationAssesses the retina and macula
Fluorescein angiographyHelps identify leakage patterns
Indocyanine green angiographyAssesses choroidal changes in selected cases

Central Serous Chorioretinopathy Treatment

For the first case of CSCHR we may opt for observation as many acute cases resolve on their own. As for the follow up of CSCHR we still do it very much so when symptoms persist or come back. In terms of what we choose for central serous chorioretinopathy treatment that is based on disease duration, recurrence, visual symptoms and what the imaging shows.

For chronic CSCHR which is a long term issue an ophthalmologist may look at photodynamic therapy or certain laser treatments. An evidence based treatment guideline available on PubMed reports in support of reduced dose or reduced fluence photodynamic therapy as a good option for chronic disease which many a time do not require treatment at all for acute cases.

There is not a one size fits all central serous chorioretinopathy treatment. If CCSR is associated with abnormal new blood vessel growth that does play out differently. What we do should be based on retinal imaging and specialist assessment which in turn rules out self treatment.

Eye Care and Follow-Up at Vasan Eye Care

New central blurring, distortion or a gray patch should prompt an eye exam. Also if there is recurrent CSCHR, symptoms in the other eye or vision which doesn’t improve that also requires evaluation. At Vasan Eye Care we do retina services which include Retina assessment, imaging and care planning.

Early assessment does not mean every case requires immediate treatment. It means the condition may be watched for as a persistent fluid or complications will be recognized at an early stage.

Frequently Asked Questions

1. What is CSCHR?

CSCHR is a condition of the retina in which fluid accumulates under the macula and may cause blurry or distorted central vision.

2. Can CSCHR improve without treatment?

In many acute cases of CSCHR improvement is seen spontaneously, but it is important to follow up to confirm that the fluid is going down.

3. What test is commonly used for CSCHR?

OCT is what we use for CSCHR as it is able to display and track fluid under the retina.

4. Is central serous chorioretinopathy treatment always necessary?

No. In terms of Central serous chorioretinopathy treatment it is based on if the case is acute, persistent, recurrent or if it is affecting vision greatly.
