How SMILE Works

The femtosecond laser delivers precisely focused pulses inside the corneal stroma to define the anterior and posterior surfaces of the lenticule, along with a small side incision. The lenticule represents exactly the volume of corneal tissue that has to be removed to achieve the desired refractive correction. The surgeon separates the lenticule from surrounding tissue using a fine dissector introduced through the side incision and then extracts it in a single piece.

The cornea now has a new curvature, which corrects the refractive error. Because the bulk of the cornea above the treatment zone remains intact, structural stability is preserved far better than with a flap-based approach. No excimer laser is used. No tissue is ablated. Nothing is lifted. It is minimalist refractive surgery, and that minimalism is its main design advantage.
